What worked for me were leaving the house to study, writing note cards and studying them, and recording myself reading things on my iPhone so that I could listen to them in the car.
Are you able to review the questions you've missed? Do you understand why you're missing things? Are you going too fast? Oftentimes, that's why I miss things- reading too fast and missing something important.
I agree with the PP -- stop changing your answers! I know that's tough. One of my instructors tells a story of one of the students in the class ahead of me who had the same issue. She started wearing a rubber band on her wrist when she took tests, and would snap the band when she felt the urge to change her answer. Apparently it helped -- she just graduated!
Have you tried one of those assessments that tells you what kind of learner you are? How-to-study.com has an assessment quiz and suggestions for each type of learner. The suggestions look pretty good; I'll be trying some of them out when I return to class in the fall.

I actually set my mind to study a week before my examination, but then I always forget about it. and i came up studying a day before my examination, sometimes I cannot finish reading the topic given to us. I always rely on "stock knowledge" but it seems not so efficient. when the result's were given to us, sometimes i feel so bad about myself for I didn't give my best during the exam, I always blame myself why i'm so lazy to study and so forgetful.

Hi, I like what you have going so far. I would add pre-reading before lecture and maybe a study partner.
I make it a point to see/hear all the material at least times. But not just read it, get the information in lots of different ways. And understand the material.
What kind of learner are you? If auditory, perhaps recording the lectures and listening again. If visual draw up some concept maps/flow charts.

Mine include
-note taking (in class & individually as i read)
-practice questions (including reading rationales & going back to review in book)
-going over notes
-talking to myself aka teaching my dogs/family about whatever im reading
i do the above, yet barely pass my exams & i cannot understand why.
When i say barely, it means getting the minimum of 77% which is the lowest passing grade possible.
Any advice?
