Stressing out over first nursing assignment

Hello, I'm new to the board and I stumbled upon it needing some help!

My assignement is a Concept Map on Health Promotion

I choose the topic personal hygiene. We have had to turn in bib cards for each of our articles and I focused basically on hand washing and soap types, because honestly that is all I could find in nursing journals.

I need to have 3 nursing diagnosises (which I have no idea what that would be for hand washing?)

I also need to have three assesments for each domain, I think if I get pointed in the right direction I can do better.

This is my first actual nursing class, and I'm very overwhelmed. Any assistance would be great! Thank you!

This is likely your first of many, many care plans/concept maps. I highly recommend getting a nursing diagnosis hand book to help with this. They give the common-sense/medical diagnosis with a list of nursing diagnoses that would apply. From there you look up the nursing diagnosis and get the appropriate assessments, interventions and evaluations.

So, the risk of infection would be the diagnosis? My line of thinking then makes the assesment just bad hygiene which does not sound right. I am going to look on amazon for nursing books. I'm glad I started working on the project now, it isnt due until the end of the month.

Risk for injection is a very common nursing diagnosis. Another one is "health seeking behaviors". You should have a list of these somewhere, probably in a Fundamentals index or something. Three should be easy - there are probably a lot more.

You'll need to assess the patient's ability to understand the teaching. (Just one I can think of off the top of my head again)

Concept maps and care plans are the same thing in a different presentation/format. One book should be enough.

Try looking at books in your school library before buying one. See which ones have what your instructors are looking for.

As to how much you need for each domain, instructors usually state what they want, and each instructor is different. They may want a dx under each of three domains, then a certain number of interventions for each dx, etc. Try to pick dx that you can find enough interventions for.

BTW, the dx I gave you are incomplete. Again instructors differ on this, but a complete nursing dx goes like this "Nursing dx (such as risk for infection) RELATED TO blah, blah (sometimes this can be the medical dx) AS EVIDENCED by blah, blah, blah (this is the s/s or quotes from the patient).

Yes, the fill in the blank part is all part of a standard nursing diagnosis. Do you have a list of these in any of your books?

I'm surprised they didn't give you an example of what they want for the first time.

no, the only nursing books required for this class was the health promotions book and an nclex review book (which we never use). The promotions book is small, and very vauge. I have a nursing fundamentals book by the ATI company, but I checked it and it doesnt seem to have any of that in there, or atleast not in a way that is easy to find.

I ordered a book off of amazon, but it probably wont be here for a week.

I really feel as if my instructor is not giving us enough information to begin with. I just e-mailed her again today, but it will probably take a few days for a response. I'm trying to work in advance, but I feel stuck until I get that book.

OK, another link that would directly affect you as a nurse is this one relating to handwashing. About halfway down the page, in IV.A, it discusses hand hygiene. ;)
