If you are a Licensed Nursing Assistant (LNA) you may be eligible for reimbursement of your training and testing costs. Does anyone know if we can get a new form? I moved after I recieved my form for reimbursement, but I cant find it ANYWHERE to save my life.
An application must be completely filled out, including your full address, by yourself and by the nursing facility that hired you, and forwarded to the N.H. Board of Nursing. Please note that the address printed on the form for the Board of Nursing is incorrect and that the forms must be mailed to N.H. Board of Nursing at 21 S. Fruit St., Concord, N.H. 03301. Applications are available through nursing facilities and have the directions printed on them.
