thank you for your welcome
will attend discussions soon
technology a little new to me and currently only have access at work
if you know any body who might have a lesson plan for basic assessment (start of care data collection ) it would help me out no end
best wishes Julian

lesson plan for basic assessment
Is it a complete health history guide that you seek?
I have a chapter on this if this is what you need.

Good morning - the author is Jarvis, Carolyn and the title is "Physical Examination & Health Assessment" 4th Edition 2004 ISBN 0-7216-9773-9
There is the Elsevier Science homepage - http://www.elsevier.com with "Customer Support"
There is also a Pocket Companion with full-colour access to health history points, examination steps for each body system (0-7216-9774-7) available via the web at www.elsevierhealth.com or by calling toll-free 800-545-2522 (or so it says)
It goes through the sequence:
1. Biographical data
2. Reason for seeking care
3. Present health or history of present illness
4. Pat history
5. Family history
6. Review of systems
7. Functional assessment or activities of daily living.
Hope this is of help for you, let me know if you need more information.
