Does anyone know of some good internet resources for looking up Nursing Interventions according to Diagnosis or Nursing Diagnosis?

I am particularly stumped on finding interventions for a resident with "Thrombocytosis" which is high platelet count in the blood.

Our instructor requires 2 interventions for each medical diagnosis. The only signs and symptoms of this disease are lab values. I cannot come up with a "practical" intervention to deal with this disorder. I did see an article indicating bleeding from gums and gastrointestinal bleeding are associated with this. Help!!

Thank you VERY much, in advance!

the only cross reference i saw on the internet was in the ackley/ladwig care plan constructor (i given you a link to it below), but i believe they have changed the way the website is arranged and it may no longer do that. these two medical dictionaries have cross references in them:

what other information do you have about this resident? thrombocyctosis is something that generally does not exist on its own, but is a symptom of another medical problem:

  • severe hemorrhage
  • iron deficiency anemia
  • surgery
  • post splenectomy (transient)
  • malignancy
  • inflammatory bowel disease
  • recovery from acute infection
  • drugs
  • epinephrine
  • vincristine
  • alcohol abuse
  • pancreatitis
  • cirrhosis
  • hemophilia

thrombocytosis (secondary thrombocythemia)

thrombocytosis can develop secondary to chronic inflammatory disorders, acute infection, hemorrhage, iron deficiency, hemolysis, or tumors (see table 2: myeloproliferative disorders: causes of thrombocytosis). platelet function is usually normal. however, in myeloproliferative disorders, abnormalities of platelet aggregation occur in about 50% of patients. unlike et, thrombocytosis does not increase the risk of thrombotic or hemorrhagic complications unless patients have severe arterial disease or prolonged immobility. with secondary thrombocytosis, the platelet count is usually

nursing interventions treat the patient's symptoms, not their medical disease.

Thank you so much for the help, resources and advice!!

It is GREATLY appreciated!
