So I have to do a care plan for a virtual patient on a cite called Shadow Health. I am having some trouble with the 2 nursing diagnoses to start the care plan.
Patient is a 78 year old Korean female who was just admitted into the ER for abdominal pain and difficulty having a bowel movement. She has not had a bowel movement in 5 days and her last bowel movement was complete diarrhea. She has a history of hypertension, that is well managed with Accupril 10mg. She states that the pain gets worse when she moves or eats. When doing my physical assessment, abdomen was not distended, but tenderness when lightly palpating left lower quadrant. Also, when deep palpating there was an oblong mass over her left lower quadrant.
They did not give me any lab values or medications she has had besides the Accupril.
Any help I would greatly appreciate it!

Nursing diagnoses are made from nursing assessments, medical diagnoses are made from medical assessments. To make (not choose or pick) a nursing diagnosis, you match your assessment findings with the defining characteristics of a nursing diagnosis, not the other way around.
So what asssessment data do you have on this lady? List them in outline form, then see what that suggests to you. Check possible nursing diagnoses in the NANDA-I 2015-2017 (NOT a "handbook") and see if you can make a diagnosis that way.

I 'm gonna help you with this one but don't tell anyone, ok?
Look again keep this on the QT but hear tell many Asians are relativly more predisposed to stomach cancer than people of other cultures because they are said to have more pickled foods in their diets.
