Does anyone know where I can find a sample care plan for "Ineffective Sexuality" pattern? I need identifiable and measurable short term goals with corresponding Interventions, and Ackley's online Care Plan constructor does not include this diagnosis (I wonder why???).

We're not allowed to use premade care plans in my school. Use your data clusters to help you come up with a related to..then go from there. Your patient is going to have different goals, and different needs than a care plan anyone will provide for you. Just sayin.

the constructor only has the most commonly used diagnoses in it. there are 188 diagnoses; the constructor contains 75 of them.

ineffective sexuality pattern is not a commonly used diagnosis by med/surg nurses. its definition is expressions of concern regarding own sexuality. i'm looking at the interventions for this diagnosis in my copy of the book. . .they read like a sex therapist's instruction manual. are you sure this is the diagnosis you want? some of the outcomes listed are (page 1085, nursing diagnosis handbook: a guide to planning care, 7th edition, by betty j. ackley and gail b. ladwig)

  • state knowledge of difficulties, limitations, or changes in sexual behaviors or activities

  • state knowledge of sexual anatomy and functioning

heh heh. no, i'm not sure i want to use this diagnosis. unfortunately, i have to. my preceptor assigned us the sexuality pattern this week and so i need to come up with a diagnosis related to sexuality. it seems the only one that fits, and is in no way a top priority. however, during our informal interview, the patient did admit to being dissatisfied with the lack of sexual intimacy since he has been hospitalized (almost 3 weeks).

so far, i've got: patient will describe three acceptable alternative forms of sexual expression by end of shift (day 1).

but, i cannot seem to come up with another...i know my interventions could include providing privacy while in hospital, encourage/facilitate communication with sig. other regarding concerns....
