Hey everybody. I just saw I received an email from SMC on Sep. 21st to take the NLN-PAX next month! Since there doesn't seem to be a thread for this prospective cohort yet, I thought I'd start one now for us to share info on the application process.

My physical exam went from approved to rejected. Did you guys send in additional lab reports that is not on the physical exam paper we have to fill in?

Mine just told me that too! It said the hearing section wasn't filled in

Also my flu shot keeps getting rejected bc it says there's so expiration date but all I have is the next dose due date I keep submitting and they keep rejecting it ?

Did you get them with UCLA? I asked the receptionist for it and it was a special paper that they had to search for

On the "ATTACH ALL LAB REUSLTS" section in the paper. Did they write anything special? mine missed it completely and I don't know how they are gonna fit in all lab results on that small section haha

Mine didn't write anything ! ? is that why they rejected yours ?

Yes it says nothing, I also had no abnormal findings maybe thats why my doctor left it blank.

This castlebranch is stressful, I had nothing abnormal also that's why it's blank, did the doctor fill out the audio gram or vision portion of your form?

My rejection was due to hearing, vision and lab report it says. My doctor only filled out Vision uncorrected but not plus sphere and tone audiogram? I don't know what that is.
