I just want to start this thread for students who are applying for the nursing program at SJDC this Spring 2018. We can share some information or ask questions while going through this process. I actually finish all my pre-requisite classes at Delta and now I will be applying with 64 points.

Does it matter if we check mark the lines or do we use our initials on the documentation submission packet. I know it's a small detail but I don't want to give them any reason to disqualify.

I have the same question as well. But I think it doesn't matter cause they didn't have a specific instruction whether to put initials or check mark.

I was also asked to provide supporting docs! One step closer.

Do you guys know if we have to send all TEAS attempts to them, or just the one we're using? I took the TEAS twice, got a better score my first try, but I still reported that I took it twice. When I go to the website to send my results it looks like I have to do it seperately. So should I send both or just the one? I'm so afraid of doing something wrong and getting disqualified!

I am pretty sure you just send the score you are using.

I have taken the TEAS V and the TEAS. When I applied to Fresno City, they asked for the TEAS score so I just sent them that one and ended up getting disqualified due to not sending in all scores. I would send both just to be on the safe side even though it will cost a little bit more. Unless you can call the office and get a definite answer on that one.

I have a little bit of problem. I requested the ATI to submit my TEAS score to Delta and got a confirmation email. On that email they attached a pdf file about the information of my TEAS score, however they put a wrong location. I took my TEAS twice and the first time was at University of Phoenix Modesto and the second time is at University of Phoenix Sacramento. I used my second attempt result, but they put Modesto instead of Sac. I'm scared that I might get disqualified just because of the location since it was clearly stated on my application that I took it at Sac.

They told me to turn in the ATI teas score I put in my application even if I have taken the TEAS V once already. I went into the Health Sciences and that's what they told me. Has anybody else had this issue?

Mursetobe I am trying to understand what you are asking. You have taken the TEAS V and the new TEAS and you reported the TEAS score because that is the one they want. You don't need to report your TEAS V score as far as I understand from speaking with the councelor. I have taken both TEAS V and the new TEAS. Not sure if that was any help. í ½í¸Š

Hi! I was also invited to send my documents! There were two pdf documents. I was able to open the first one, but not the ADN Checklist. I accidentally pressed CLEAR on the ADN Checklist. So now the checklist pdf file is missing(or deleted).

Do you guys think Health Science App would send me another copy? What was on the check-list pdf? Is it possible if one of you who have that pdf file could send it to me? Or is that check-list pdf created strictly for each person? HELP!

Congrats on getting invited. The checklist is created with your info on it so you would have to contact the school about it. I am sure it will be fine. Good luck!!
