1–4 weeks of age 50 mg/kg per dose every 8 hours IV
It is not necessary to differentiate between premature and normal-gestational age infants.
Infants and Children (1 month to 12 years):
For body weights less than 50 kg, the recommended daily dose is 50 to 180 mg/kg IM or IV body weight divided into four to six equal doses. The higher dosages should be used for more severe or serious infections, including meningitis.
in real life if pharmacy did not question it....I would call the MD and clarify how many mg/kg they want given.1–4 weeks of age 50 mg/kg per dose every 8 hours IV

It is being given Q8H. ____mg x 3(doses) = _____mg/24 hours.
The dose being given is cefotaxime 270 mg x 3 (every eight hours) so the total dosage ordered in 24 hours is....270mg x 3 = ______mg/24 hours.

How do you determine safe dose ranges for cefotaxime 270 mg via IV piggyback.
Sterile water for injection 5.4 mL concentration = 50 mg/mL, Q8H?
Infant weights 5kg and is 30 days old.
I found different safe dose ranges, but I'm not sure which one to use.
