Anyone in Rural Health Facility or HPSA? I am considering moving to this arena for loan repayment purposes and would like to hear from someone already in it. What's the situation, how are the hours, call, compensation, benes, patient load....etc etc...???

The people I know there now are not midlevels, but in other positions, so I don't know exactly what the midlevels started at...but very good point not to accept that just because you are a new grad. If one is going to work hard, on top of all the loans and time spent on school, there should be just compensation. So what do you think would be a more accurate amount for a new grad to start at? Have you any figures from elsewhere?

Being a new grad you don't have much negotiating power unless they are in desperate need and no one else wants to work there. Your best option as a new grad is interview with several and get 5 or 6 solid offers and use these numbers to get the best deal. Then, watch your numbers and pay attention to how much you are bringing the clinic every month. If you bring in $10K/month, you're probably only worth about 50K/yr. But, it's easier to negotiate with the next 20-30K/month as the overhead is already met w/ the first 10K. Now you're just negotiating where the profit goes. If you bring in $40K/month, you should easily make six figures. If they won't budge, just keep track of your numbers so you can show a similar clinic your profit margin and if you're hard working, come across as smart and responsible, and play your cards right, someone will be happy to give you the deal you want as they stand only to make money too. Would you rather have a NP working for you that cussed every time a patient came in and was rude and short w/ the patient because they make $60K/yr no matter what they do, or would you rather have a NP who is a hard worker, treats the patients like a family member, and busts their butt because they know they'll make a fair share of their gross collections? The 60K NP will probably bring in about $150-200K/yr, so you get $90-140K from them. The other NP on productivity profit sharing is probably costing you twice as much, but bringing in $400K/yr gross collections. 400K minus 120K is $280K for you, the intelligent clinic administrator. $280K with happy patients or $90K with patients who hate your clinic? That seems easy enough to me....

Thanks for the encouragement. I hope they see it that way, but spelling it out and actually getting it are two different things. They may just say, all that sounds good, but the average NP makes $70K or whatever. We'll see... I get to drive for 5 hours tonight after work to meet with one of them tonight. Yeeeeeha.

Well, after countless emails and phone calls, it's down to two choices. Missouri sucks as all the RHC/CHCs want to pay $55K/yr. I asked them what they want me to do for the other six months of the year, and they didn't think that was funny like I did. :o)

I am visiting with the last two over the next two days, and get to drive about 20 hours over the next two days as well.

But, the offers are much better than this worthless state. Can't say I'll be sorry to leave at all, at least from a professional standpoint. The pay is terrible, and the law is just as bad (no CS rights and mindless geographic requirements).

Thanks Siri... Too bad I'm not getting the ER offer like you!!! They're offering $20K/yr for 1:4 ER call; granted the ER only gets 3-4 visits/day.
