I am doing a paper on a risk mgmt. The occurence needs to be reportable. If anyone can help me? it would be great. I am interested in an IV topic. any ideas??

Reportable to whom? The hospital risk management team? The local health department? The CDC?

One event I read about comes to mind. A man was treated for phlebitis from an alleged peripheral IV placed on a previous admission. On the readmit he insisted that the nurse had not used sterile technique (I hope Im making sense). The old records were obtained and no where had anyone addressed the man having an IV started, the site of any existing IV, and several of the abx doses were not signed out.
I think I read this in Nursing 200_ but Im not sure. I might have been on Medscape.

How about the accidental free-flow of IVF (due to pump malfunction and/or human error) such as Pitocin and/or MgSO4 resulting in obstetrical emergencies and deaths.
thanks, I will research that, it sound like a good one. I need 2 interventions also to go with that. How about pump maintenance and observation every 15 mins

Also, IV push phenergan and extravasation has been much in the news lately.
