Did I write my nursing diagnose correctly?

Risk for infection RT uremia resulting in decreased immune response, use of foley catheters, picc lines, malnutrition, decreased h&h, hx of UTI, administration of corticosteroid and wound L. foot secondary to acute renal failure, diabetes mellitus.

My instructor isn't big on the Risk for diagnoses, but I thought it was the only diagnose that was less complicated for me.

and my goal

Client will remain free of infection, as evidenced by normal WBC count, temp

in your rt you can't use any actual medical diagnoses....such as diabetes.

can't i write secondary to?

This is what I have so far.

Should I write by labs in the RT ?

GOAL:

lient will remain free of infection, as evidenced by normal WBC count, temp

Assessment 10/01/08

1. Assess for presence, existence of, and history of risk factors of infection.

2. Monitor white blood count (WBC)

3. Observed/Monitor for signs and symptoms of infection.

4. Assess for nutritional status.

5. Obtain appropriate tissue/fluid specimens for observation and culture/sensitivities testing.

6. Assess immunization status.

Decrease stressors: 10/02/08

4. Stress proper hand washing technique by all caregivers between therapies.

5. Encourage deep breathing, coughing, and turning q 2hr.

6. Provide regular catheter/perineal care.

Teaching/counseling/referrals:10/01/08-10/02/08

7. Teach family members and caregivers about protecting susceptible patient from themselves and others with infections or cold.

8. Teach patient and caregiver the signs and symptoms of infection, and when to report to physician

9. Review individual nutritional needs, appropriate exercise program, and need for rest.

Assessment

1. Assess clt at 7:30 am. and clt has a Foley catheter present, wound on the L. heel, PICC RAC and Hx of UTI as a risk factor.

it would sound better as risk for infection r/t inadequate secondary defenses, immunosuppression, invasive procedures, and malnutrition.

your goal should be a reflection of your interventions.

My patient isn't able retain information. (short memory loss) If I taught him anything it would be use less. how do I get around this.

Put the patient on a routine. Instruct the staff to maintain the routine.
