can anyone advise me if I have incorrect times that I wrote on an intake sheet {on paper in PACU can I rewrite on a new sheet? The patient has not been discharged. The original times were incorrect due to a clock error? Must I leave original sheet in chart or put in shredder?

Cant you just cross out the incorrect time and write "error" and initial, and write the new correct time by it?
What is your policy?

You could ask your manager what is preferred in your place. If I didn't have that information I'd be inclined to just make a note on the paper itself that says something to the effect: "Note: All above times discovered to be fast by 5 minutes due to inaccurate clock" or something similar. It doesn't seem completely appropriate to recopy but that's just because I don't think it looks great overall, not that it's technically wrong (as far as I know).
Or you could line through each time and write the correct time, initial each one. Then make similar note at the bottom "times above corrected to account for clock inaccuracy (fast by 5 minutes)."
