So I'm looking into transitioning to utilization review, my experience is pretty much only in hemodialysis.
My resume's one page. Currently I have my job experiences with its responsibilities, education, and certifications (basic BLS, license, and a conference I attended in 2013).
Is there anything else I should really be putting in it? / How do I cater it to a utilization review position?

In almost similar situation. Most of my nursing experience is in HD and shooting for your job in a year.
