Reactivation of expired license

I had a previous PA RN license. We moved out of the state two years ago and I foolishly allowed my license to expire. We are now planning a move back to PA and I am trying to get in contact with the PA BON to get a reactivation application (per all the information on their site, you must call in to receive this, it is not online). I have called many times, spoken to someone twice who says they will send it out, only for it to never arrive. I have sent several emails that go without a reply and even sent a fax!

I was wondering if anyone else has been in this situation and what they have done to finally get the reactivation application they needed. If so, by any chance does anyone else have a copy of the application? I am at a loss and don't know what else I can do.

Follow up:

I ended up going to the PA BON in person to get the reactivation application. It took only a few minutes and I had the application in hand. So, if anyone else has trouble like this and they can get to the area, they might want to drop in.

I have had to go through this same process and have turned everything in to the BON and have not heard back now for 3 weeks, do you remember how long it took to reactivate once you sent all the paperwork in?
