i am new to this site have no idea what im doing, i am a new rn student working on a care plan for risk for deficient fluid volume and my teacher shredded it where can i find some help or new ideas

did the teacher shred your work accidentally or on purpose? you can find a lot of information about care plans by reading these threads:
i will check for students posting and asking for help with care plans almost every day.
the definition of the nursing diagnosis of deficient fluid volume is "decreased intravascular, interstitial, and/or intracellular fluid. this refers to dehydration, water loss alone without change in sodium." (nursing diagnoses: definitions & classification 2005-2006 published by nanda international. page 79.) two students have posted threads looking for help with care plans related to patients with dehydration in the last day. here are links to them:
when you post a question it is helpful for me if you give me information about the patient, whether this is a real patient or a case study, your pertinent assessment data, and what has you stuck. all care plans begin with sorting through the data that you have collected. nursing diagnoses are chosen based on that data information.
