I just received an e mail from my manager informing me that I have been asked to attend an RCA meeting regarding a patient I had last week. The patient coded, was worked on and sent to ICU. although I am not 100% certain, I feel the situation likely did not have a good outcome. I understand that the point of the RCA is to analyze the situation and collaborate to come up with ways to keep it from happening again, and that it is not designed to place blame or get anyone in trouble, but I am still feeling a little nervous. I am still a pretty new nurse, I got my RN license almost 8 months ago. What Can I expect from this meeting? Any tips or advice??

For RCA on your patient, it doesn't necessarily mean that the pt had a bad outcome, but maybe there were other events that could have lead up to the code and transfer. it is possible that the hospital does an RCA on all codes with transfer to ICU. maybe there was a missed diagnosis, etc.
They are great learning experiences because it will also have the physicians/NPs, Respiratory, all others who have cared for this pt and their viewpoint, plus you will hear before and after code analysis.
This will greatly open your eyes to the 360 perspective of care of a patient, not just the RN role, but how physicians, dietary, respiratory and others interact with each other.
