It is a tough situation to be in . AND you are not doing her a favor by doing her work.

Talk to her talk to her supervisor. I have no doubt you will be tactfull express your undrstanding as you have so beautifylly here.

Her supervisor does nothing because you are doing the work and she, like you, has a soft heart for this person. You need to do your work and put pressure on her supervisor if that means letting everything go to H in a hand basket then do it. It is her responsibility to see this woman does her job or hires someone else. As long as you keep picking up the slack she can hide behind the soft heart that she has for this woman.

Unless you intend to claim this disable nurse as a personal tax dependent do not provide her a free income. That is precisely what you are doing. Are you sympathetic enough to financially support her yourself from your pocket? This is what you are doing by doing her job while she not you get paid for it.

You are not callous you are simply pointing out the emporor has no clothes.

What bothers me is that the Director (ie her boss) is aware of what's going on and is doing nothing. Should I make an appointement to talk to her as well.

This makes me sound like such a door mat I know. I honestly feel like people are going to hate me if I start making a fuss about this. I'm usually really brave about dealing with difficult situations, but this one is so difficult.

Steer clear of this situation. I have seen many get sucked into a problem trying to resolve that probelm with the outcome of becoming part of the problem. Sometimes it may be best to improve upon our own abilities becuase we all fall short in areas. Unfortunately management are human too and need support of subordinates instead of undermining behavior. It is best to maintain a positive attitude despite what the current situation holds. As an ex manager of a full service restaurant, I can appreciate those who support me when I feel I couldn't go on. The making of a good manager is by those who work with the manager. Try offering to help out any way possible and the rewards of your committment will be exceedingly great. Trust me. I know.
