Questions about Chronic Disease Case Management

Hi. I understand the role of inpatient case manager, but am interested in chronic disease case management. What are the day to day activities in this role? Do you work with other disease management nurses in an office setting?Are there quotas/metrics by which your job performance is measured? What kind of personal traits would best fit this type of job?

Hi Pappilon-Are you confusing me with someone else (or was a post I made form a long time ago)? I don't remember a patient on hydroxyurea scenario. Please refresh my memory (I am tired also, so maybe I forgot?)

I currently do telephonic disease management in a heart failure program. Yes there are metrics. We have to prove that our program is valuable by how many contacts we make and documenting all the teaching we do.

You have to type and talk at the same time.

Check with insurance companies if you want to be a Disease Manager. They focus on cardiac disease, DM, COPD, renal illness - chronic diseases that we can positively influence. Purpose: cut down on ER visits, cut down generally on insurer expense related to these illnesses.

Do you follow a script? What if patients don't make lifestyle changes - is that held against you? It seems that just counting numbers and documenting what you teach isn't a real good way to measure change, which is what you're really wanting to do, right?

What path would you take if you wanted to get into actual case management? Thanks for any help.
