question about staging wounds

I had resident with decub on ankle. Resident also had had surgery in past and had hardware in this foot and leg. About a week after area began, hardware was showing through. Resident was sent to see surgeon who removed hardware. What stage should this wound be put as. Surgeon also told family that the screw was completely out of plate on outer ankle and that had caused the sore to occur in first place.

I would not refer to the ulcer as a pressure ulcer. It does not sound as though pressure caused the area. The hardware probably caused the area. I would read all of the documentation from staff and the MD's to get the correct info on this area. I would be interested in hearing what everyone else thinks.

Yep....not pressure ulcer. Think of heels, coccyx....anything that can rest on a surface long term is "pressure".

Ankle wounds can be considered as pressure wounds but this wound could be considered as a surgical dehissed wound or a full thickness wound
