Question please help!

Hello! I'm doing my pediatric rotation this semester. My patient this week is a 16 y/o diagnosed with hemophilia in 1997. His reason for being in the hospital this visit is due to a hematoma in his right knee. He was recently hospitalizaed in july for an inguinal hematoma. At that visit, he received Factor 8 Recombinant BID and was sent home 10 days later. This visit, he is also receiving Factor 8 Recombinant BID. A PICC line was placed in his right arm 2 days ago and will be going home with it so he can receive Factor 8 Recombinant at home daily. He was complaining of pain the other night and has been on bed rest to help him heal since his admission. He cannot bear weight on his right leg either. PT will be coming for crutch training I think. My question is about choosing some nursing diagnoses for him. I have to choose a physiological and a psychosocial. I was thinking about doing "Risk for injury r/t deficiency in blood clotting and impaired mobility" as my physiological, and "Risk for situational low self esteem" or possibly "Loneliness" as my psychosocial. What do you think? Please help! I would greatly appreciate it! Thank you in advance and have a wonderful day!

I think your physiological ND is great. For the psychosocial you'll have to ask him how he feels about his disease, how he is coping, how he feels about being on crutches for the next few weeks. Then you'll know if he is suffering from low self esteem or lonliness or powerlessness.

i think i would prefer an actual nd, rather than a risk for.

1. imp mobility r/t pain, nwb status of r leg.

2. social isolation r/t recovery process.

I think it's great to be able to help out - but just a heads up - you might want to reword future posts - I could see how this could be construde as a HIPPA violation - too much information. Maybe if it were worded as a fictional case study instead of a real patient. Just don't want anyone to get into trouble. :) Good Luck in Nursing School!!! :)

I am sorry but I don't see how she is violating HIPPA. She has not identified even the part of the country she is living in, let alone what state or city. She has not identified the hospital by name or size, if it is for profit or nonprofit. If someone is possible of violating HIPPA, I have when I posted about a former governor, his brother the governor and the fact that I once lived and practice in NY.
