hello!!!

i need all the help i can get....

where on earth can i find qualpacs/ quality patient care scale? i've been searching for days now... to no avail.

please help me!!! i really need to know what this form looks like.. i need it for my research project.

any help would be greatly appreciated!!!

This sounds like a proprietary thing (something developed and owned by a private company that sells it to facilities to use). I would recommend that you call around to some local hospitals, ask for the nursing department offices, identify yourself as a nursing student doing an assignment on this and ask if they use this particular system. If they do, ask them if they would be willing to let you have a copy of the form. At the least, ask them if they would give you the address of the company so you can contact them for information. If you were here in the U.S. I would tell you to call the 1-800 telephone operator and ask to see if there is a national telephone listing for a company called QUALPACS. Another thing you could do is search the Internet resources in your country for "patient acuity (or quality) assessment tools" and see what comes up. If this is a commonly used acuity assessment tool in the Philippines, it's bound to be mentioned somewhere along with the name of the company and it's location. Good luck!
