Quality of nurses?

I've been a nursing supervisor for many years, and maybe it's just me, but it seems there has been a gradual decline in the quality of nursing education - possibly education in general. I'm appalled at some of the spelling and (lack of) punctuation I see in documentation, and even more alarmed at some of the ignorant things that I see written in charts.... "applied barium cream to sacrum"...."instructed patient that if she cries without tears she is dehydrated".... Just a couple examples that make me feel embarrassed for my company and for nursing in general. Is anyone else seeing stuff like this? Or am I just being too picky? :confused:

I just discovered a diabetic patient who didn't have his feet assessed from SOC and 2 revisits. Pt was referred for wound care. :eek:

I remember working with a nurse who spoke English as a second language. Her handwriting, as well as her use of English in general, and medical charting in particular, was atrocious. Even the nursing supervisors spoke about her poor nursing notes, but I never saw anybody take her aside and give her constructive criticism or help to overcome her problem.
