Pulling right med from wrong patient in pyxis

The right medication (Ativan) was pulled from a different patient in the pyxis. This was was the correct medication there was no harm done however it was not reported. Will this turn into a larger issue if a discrepancy shows up that this medication was not given to the patient it was pulled from in the pyxis?

I would report it to my manager

right med ordered given to right patient,

unfortunately pulled under wrong patient, no alarm it was not ordered for patient it was pulled under,

it it was an accident....it is a wonder you even noticed......

but since it was a controlled substance, that is why I would report it...

i I know how to fix it, pull med under correct patient, and return under the patient it was originally pulled under, but unless this is done within the hour....sorta iffy.... Thus another reason I would report to my charge, or mgr.

Please do not wait to report. Due to the nature of the med, it is essential that this be taken care of at the reporting of the one making the mistake, not simply found after the fact.

And yes, of course it will be found. Charge will run a report and find the discrepancy, likely at the end of shift.
