That would work on a person not addicted to drugs....Won't make a bit of difference except in your own head. What they actually heard was that they have cancer and now when they come in they need more and better drugs.

Most of our cyclic vomiters are related to marijuana use. There are actually some good articles out there that you can find to read about it.

In my ER, we're required to do preg tests before giving Toradol. Does anyone else do this?
We used to be able to do dip urines and hcg in the ER way before CLIA regs. Not anymore. If your lab is taking 4 hours to get labs back I would be writing a vriance each and every time. That is not stat and can be way dangerous. There should be no reason especially on a test that is complete in under 2 minutes.

If you check your drug references it is contraindicated in pregnancy. We do ucg first. Toradol is also contraindicated in early pregnancy. FDA pregnancy category C. This medication may be harmful to an unborn baby. Tell your doctor if you are pregnant or plan to become pregnant during treatment. Taking Toradol during labor can increase the risk of bleeding during childbirth. Do not take this medication during pregnancy unless your doctor has told you to. This medication can affect fertility (your ability to have children). Do not take Toradol while you are trying to get pregnant. Toradol can pass into breast milk and may harm a nursing baby. Do not take this medicine without telling your doctor if you are breast-feeding a baby. Do not give this medicine to anyone younger than 18 years old.

pyxis dilemma
You can also be sure that pharmacy hasa system in place to see how much medication that this nurse is taking out. If all of your facts are true you will actually place your own license in danger by not reporting. anonymous isn't going to work later, everyone might decide to claim that they were the anonymous tipster. You can call your ethics hotline. Umm and palliative care is an awesome place to be for someone with a diversion problem............ She may not have pyxis access, but these pts may be on pca devices, some folks will actiually steal pain patches right off he patients. Or how about. "You look busy, how bout you pull out some pain meds and I will give those for your." Then her name appears no where and the documented care giver take the fall in the end. Oh and people who NEED the meds aren't getting them and suffering.

oh i would have had "a little chat with him," it would have had nothing whatsoever to do with a breakdown of my patient load. i would have informed hime that i understand that you are unwell but at no time is it appropriate to speak to me in such a fashion. i will take very good care of you, you will recieve very good medical care. your part of this relationship is to be respectful no if's and's or but's. i have had this conversation with many patients, i do not care how old they are.

We do this to some extent. The patient will be brought to a room by either a tech, or a nurse. Either will at least get vs, give them a gown, and sent to collect a u/a if applicable.

drug testing
Pretty much every employer requires a drug test for any worker related injury. The insurance companies require it. Presumably so that if the owrker tests positive they can state that may have caused the error that led to the injury.

There are so many "interesting out there" I had a patient a few weeks ago who sent her husband to town to try and buy her a foley catheter because she "felt too weak to walk to the bathroom."

What about those people who pick their nose while driving their car? Where do you suppose they're going with those nasty fingers, and do you think they're going to be washing their hands before handling those tomatoes? Oh Thanks. Now I will never be able to eat a tomatoe ever again. I agree. To the Original poster do you believe that all of those other non-scrub wearing folks are sterile? These are the people spreading contagion, not me.

What did I see???
That IS pulmonary edema. Think pink frothy sputum. IV Lasix, bipap, or intubation this would have given some peep to try and force some of the fluid out of the lungs. Pressors also possibly.

What are your bicarb abd BE? You are dealing with a metabolic problem that affects the respiratory, versus a respiratory problem that affects the metabolic system.

Anyone use Ntrous Oxide in their ED?
We used to use it in one of the ERs that I worked at years ago. We would use it for things like burn debridement, fx and such. I havve read some articles that it is popular in many peds ERs for such procedures also.

blood in stool but...?
If your pt had been drinking artificially colored red drinks like Hawiian punch or Hi-C they can have what appears to be frank bright red stools that are NOT blood. It is the dys from the drink.

Bullying in the workplace
My facility has a very concisely written policy written on its non-tolerance of lateral violence. It also covers that subtle stuff such as body language, facial expression, etc. Fortunately my department is a great group and I see very little of this here. Now there have been places that I have worked over the years that I would have loved to see something like this in place.

When is a difference in standing and sitting pulse normal?
You may be referring to a tilt test. This is when you take a patients pule and bp in a lying position, then in a sitting position andd then in a standing position. An increase in 15 or more indicates that the patient is probably dehydrated and needs some fluids.

St John is a good employer, not the highest paying, but a good employer. Some of the higher paying places are not so great places to work. I doubt that you will find another hospital willing to pay off your debt to St John so I would be sure that it will be a place you want to work before committing.

Trauma Naked?
I am going to go out on a limb and assume it must have been a signifigant MVC to have the trauma team activated We do not work all MVC pts as a trauma. I will say the same thing about an immediate u/s. We will always cut the clothing off a suspected or known trauma patient. We need to examine every inch of a pt to make sure nothing is missed. Usually an effort is made to quickly cover a patient with warm blankets to prevent chilling. It can indeed seem like an eternity when you are the one layed out on the table. For everyone coming out to bust the chops of the staff, let's remember that OP stated "It felt like an eternity". Could it have been a matter of minutes or even moments???? I have seen a pt stripped, head to toe visual, foley and abd u/s done in less than 5-10 minutes. Remeber the patient who sued over the trauma rectal exam believed he was alert and conscious but had sustained a head injury............
