Problems in the OR

What are problems you frequently come across in the OR that is annoying? (excluding interpersonal problems).  For example, retained needle or sponge, irrigation on the floor etc....

Situationally unaware personnel. Hyper focus on instruments and sponges and sterility and complete blindness to specific anesthesia and surgical issues. The scrubs consistently get it....RN's not so much. RN's need to spend 2 weeks with the anesthesia provider when training for the OR....some don't even spend a day...

Second this. In my teaching hospital I have suggested (and been ignored) that there should be a time out for induction and extubation of patients. Without it there is literally no surgical procedure happening. Some RNs, techs, residents, doctors, surgeons I have seen on the moon in personal conversations while their patient is across the room being inducted. The only place I want to be while my patient loses their muscle tone and ability to breathe on their own is next to them while it happens. Wild to me.

I think my pet peeve is colleagues not tidying up after themselves when their cases for the day are done . Frequently I come in the next morning and find drawers stashed with items that were pulled but not used, just thrown into empty cabinets / drawers in the room (which are supposed to remain empty) , supplies left on booms, towers , window sills and generally the room in a mess (yes despite it having being "terminally cleaned" , which is another peeve altogether) . Yesterday I found Allograft (probably worth $20,000-$30,000) just tossed in the unused supply return bin . Basically someone just couldn't give a damn about taking it back to the Biologics room ; being lazy.
