Policy writing?

Can anyone give me some guidance on policy writing?  I work for a small infusion company.  We previously had a DON and now no longer operate with one since we only have 3 FT nurses.  I discovered some of our policies are lacking.  

What are policies based on and where would find this information?  I have never held a position where writing policies was one of my job duties so I don't really know what I'm doing or where to start.

My Radiologist-boss's advice when I first was tasked with writing department policies: write what you do.  I would also review older policies, and perhaps policies from other departments, for format questions and the general "feel" of what to include.

A lot of organizations have policies that you can find online through a Google. No need to reinvent the wheel. 

When I worked in homecare I went online and found policies from other agencies and used those as templates and tweaked them to our needs. It worked really well.
