RA is a systemic disease, OA is localized. RA has an earlier onset than OA. RA starts in the smaller joints, usually bilaterally and symmetrically. OA usually starts in the weight-bearing joints (hips, knees) asymmetrically. Patients with OA are RF negative in labs, while patients with RA are usually RF positive.
