please read the information on https://allnurses.com/forums/f50/help-care-plans-286986.html - assistance - help with care plans. i am not convinced that you understand what i just did. this has only taken you through the first two steps of the nursing process in writing this care plan. i am concerned that you still do not understand the next step. i help a lot of students with care planning and it generally takes doing several of them before students begin to understand the complicated processes at work.

had i been your ci i would have directed you to read about cvas (strokes) and to look at all the possible signs and symptoms a person with a stroke could come into the er having. the range is broad. this poor patient happened to have the worst outcome. many others, however, survive. so, what have you learned about how to take care of them? about how to diagnose them? it is one thing to perform in the clinical area. it is another to use your intelligence and thinking to reason out answers. as an rn you will be expected to be able to reason out those answers and know why you are doing things for these patients.
