Physical Assessment?

Hey everyone! I was wondering does anyone have a really good phyical assessment form or idea? At work I am fine doing my physical assessment but during clinical I guess I get a lil more nervous and seem to forget something every single time. My school gave us one but it's not extremely helpful and seems to be kindof scatter brained to me. (lol shhh. don't tell em I said that lol). I'd like something I can print and laminate and maybe fit into my pocket.... but anything or any ideas would be really helpful for now. If I happen to come up with a good one I'll come back and add it.

if you have a copy of taber's cyclopedic medical dictionary you will find an assessment form of about 7 pages in the center of it under the entry for "nursing". the problem with assessment forms is that it's hard to find one and no one wants to share what they have. try to get a blank daily assessment form from the hospital where you do your clinicals. staff nurses usually have input into those things.
