Anyone in this forum know about pay rates (per diem, fee-for-service, or salary) and who are the leading providers of home health care in Phoenix and Tucson? Do these providers offer mileage reimbursement or other perks?

A friend of mine works for Arizona Home Care in the central Phoenix area. They are supposed to be one of the better home health companies in the valley. I don't know how much they pay or perks. They provide mileage reimbursement. You can check their website at http://www.azhomecare.com

Phoenix and Tucson are actually two very different markets, and things are done differently in both locales. Some of the agencies also do home care down there, but some have more and some have less.
