Peripheral line infiltrartion

I need immediate help for an iv infiltration with sodium bicarb.the baby is acase of early neonatal sepsis who is in severe metabolic acidosis, who is quiet unstable with severe thrombocytopenia.Central line insertion was failed and had a big blister from extravasation.Please help out .

Without having looked it up, all I can think is that maybe it's a smooth muscle relaxant. Hence it could be used for both PTL and PPHN.

The sildenafil gives them such a headache, though. Z used to cry and cry, and it took us days to figure out that tylenol actually worked better than bolusing his Fentanyl drip.

He also had the, uh, other side effect, which was pretty weird.

Do you guys across the pond use inhaled nitric oxide therapy for PPHN? (iNOtherapy)

Yep we do. We don't have the facility in my unit and would have to transfer the infant to our sister regional unit. Have worked in other units were NO was used sometimes sucessfully sometimes not.

We use nitric in the NW corks, not heard of magsulph being used at all.

MgSo4 is a pulmonary vasodilator because of this it can cause hypotension so you end up having to infuse inotropes on top. Hopfully we will start using NO in our unit soon.:yeah:
