I'm beating myself up. I've been doing my best to take care of these residents, but still I have 4 people with UTI. What's wrong with me?

Nothing is wrong with you. UTI is common in the elderly. I've heard many docs say that probably over half the residents in a LTC will come back positive for a uti because of colonization. The elderly don't drink much fluids and many are left incontinent. This does not reflect you at all.

I would just make sure everyone who's working with the residents is washing their hands and/or using gloves and hand sanitizers between each encounter. That, and with elderly persons who experience frequent UTIs I'll write a nursing order for hydration measures such as a full glass of water or other non-caffeinated beverage with each of the four med passes (assuming they're not on a fluid restriction or have other contraindications that would make pushing fluids risky).
Good hygiene practices and hydration are the key to reducing the incidence of UTI, but you'll never be able to prevent them entirely because there are so many factors that make older people (especially females) prone to these infections. Don't be so hard on yourself, OP. :hug:
