Pediatric care plan

I am a 3rd semester RN student and I am needing help on my care plan. I am having trouble with client outcomes they have to be specific, measurable, have a time frame, and be realistic and client directed. I have my nursing dx of Impaired skin integrity and my nursing interventions. If any one could please help me and give some advice I will be completely grateful !!!!! :uhoh3::uhoh3::uhoh3:

Think backwards. You jumped right to the diagnosis of impaired skin integrity in your post, but something about the child gave you the idea that the diagnosis fit your patient. What was your assessment data that made you pick that?

Often, your goals for impaired skin integrity are to eliminate or improve the abnormal assessment data. Any damage to skin should decrease over time with your nursing interventions, so often your goals for impaired skin integrity are to eliminate or improve the abnormal assessment data. After you implement your nursing interventions, what do you expect to see upon evaluation if they are successful?

That reversal or improvement should be specific and measurable (you already know what to look for in improvement because you just assessed the patient for the problem). The time frame should be realistic too based on how long you have to evaluate your interventions. For a severe problem, eliminating all the issues would not be realistic, so use an interim goal of reduction in size, color, pain. All patient problems are not solved within a single shift!

Making goals patient-centered is a matter of wording. "The patient's skin condition will improve a/e/b...."
