PCT "brain" sheets?

So I was trying to find some brain sheets that would work with my situation, or at least something I could quickly customize. I saw a ton of nurse related ones, which are great, but just don't work as well for me.

I work on a busy med-surg/hospice floor which can have up to 20-21pts (though it usually stays b/w 18-20). I work evening, so there are some days where I have all of the pts. I can print out a roster, but I am getting pretty tired of trying to search the margins trying to find or interpret my child-like handwriting for a pt's b.g/vitals/etc. Does anyone have a brain sheet that they wouldn't mind sharing with me? My situation is pretty specific, but I would still love to see some other types of examples if anyone has them.

I'm going to try and start making one....but I'm challenged with these sorts of things, so my computer may just go on strike by the end of it all ;)

I can have 4-16 patients, depending on which unit I work on. With 4 being organised is easy, with 16 not it's hard. So I will be interested to know how you all manage it, too.

This is a good topic. I'd like to know how you guys keep organized as well. I'm starting as a PCT soon and there is no way I'll remember all that stuff for 8 to 12 patients, even more.
