Now that bundling is going to be a reality in a week, what changes will your unit implement? I work for a small independent unit. We were told 2 weeks ago that our owners are in negotiations to sell out to Davita. to any other independent unit, what changes are you making?

I think we will see the end of many independent outpt HDUs. My bet would be that Davita and Fresenius end up being the big boys in the dialysis game.

Changes that will affect your pts:

1. They will need a primary care provider because only dialysis-related labs will be drawn at the HDUs.

2. Many may end up with PICC lines if they require antibiotics for something other than dialysis - related infections.

3. In the years to come, the big guys in the dialysis game will all have pharmacies that will dispense oral dialysis meds like binders, sensipar, etc..

There will be more changes as the rules change. It will be important to ensure our pts are aware and it will be important that they know whats going to happen.

Bundling ????? explain ???? I work for one of the Big Boys, as a tech. Will this be information I should make aware to to my pt's ???? most only receive Medicaid, will it effect care I provide??? give me the heads up.

There have been so many changes since bundling came in I hardly know where to begin! All I know as a dialysis RN that not all I would consider to be better in the patients interest.

We just found out this week that oral calcitriol is part of the bundle so that means it has to come from the HDUs pharmacy.
