Obtaining your Tenncare/Medicaid provider #

I looked on the Tenncare website and could not determine which form to fill out, possibly 8851, to obtain my Tenncare/Medicaid Provider #. Has anyone else encountered this issue when trying to apply for a tenncare provider #. I am an LPN. I already have my NPI #; that was quite easy to obtain!

When you go to the Tenncare website, you would click on " for providers" and then "provider person registration". Hope this helps!

I did that but it listed all sorts of titles exept LPN. Thanks for the reply. Think I will give them a call on Mon!

I am so frustrated! Tenncare emailed me and said that LPNs cannot enroll to be a Tenncare Provider! I don't think that is true! Are there any LPNs out there working indepenently and billing Tenncare?
