Hello All,
I am having the hardest time coming up with a nursing diagnosis for my patient please read and see if you can help
20 y.o pregnant female, estimated gestation 35 weeks and 5 days. previous abortion 10 weeks, had positive RPR went from 1:256 and is currently 1:32. Pt is anemic and verbalized not taking iron medication. Also stated she does not take her prenatal vitamins regularly. Maternal grandmother and grandfather had diabetes 2. Urine analysis was done also results were negative for glucose, and nitrites, trace of protein, and large amount of leukocytes. Non stress test done and revealed no abnormal results.
I need two nursing diagnosis and came up with one already: Deficient knowledge r/t lack of adequate nutritional intake aeb pt stating she is not taking her prenatal vitamins consistently or her iron supplement.
If you can come up with another nursing diagnosis it would really help.

i don't have a book in front of me and it's late right now (so i CBA to look it up) but i think you should have a physical dx for the anemia, like nutrition less than body requirements or risk for infection. the deficient knowledge is okay but honestly since (i'm assuming) a doctor or nurse had already explained the iron supplements and prenatals to her, it'd be more of a non-compliance dx.
