IV terbutaline

  1. Any policies on IV push terbutaline for intrauterine resuscitation?

I have not yet been able to find any. Any help would greatly be appreciated.

Have used iv push terbutaline (0.25 mg) to alleviate uterine hyperstim.
Have had good results thus far . I'm not aware of any written policies on giving it iv though.

I've never heard of terbutaline being given IV push either. Does it act significantly faster IV than sub-q? I would imagine that side effects would be greatly increased with IV administration, and that may not be worth the few seconds of time saved. This may be a tough question to research. Since terbutaline is not FDA approved for treatment of pre-term labor or uterine hyperstimulation, the manufacturer will probably not be of any help. You may have to search the literature for an article or study describing this use, or contact referral centers to see if they can offer any guidance.

What are the guidelines for giving terbutaline to non-pregnant patients with asthma? Is it ever given IV during an acute asthma attack? If so, what are the safety precautions. If not, then I wouldn't want to be the one responsible for giving it IV to a pregnant patient.

For your own legal protection, I would insist that the doctor or midwife administer the drug, since you have no formal policy.

I BET MARK!!! that was what I was gonna say...you better watch adverse reactions as they are likely to happen MUCH faster than w/the SQ route.

I have given it IV when needed FAST like cord prolapse, severe hyperstimulation with prolonged decel etc. Where I used to work we acutally had a protocol for IV drip terb for preterm labor, I think I only used it twice in over 5 years!
