Nutrition assessment help please

Our assignment was to interview someone regarding their nutritional intake for an average day. We then had to record their diet based on the food pyramid and compare their intake to the recommended intake from the pyramid. Then, we do a general assessment on client by body system to evaluate for signs of good/poor nutrition. We then have to come up with a nursing diagnosis, goal statement and 2 nursing interventions. My client is a vegetarian and I am going off of the vegetarian pyramid. For my client the potential problem areas I see are:

Dairy products consumed 1 serving recommended intake is 2-3 servings

Fruit consumed 0 servings recommended intake is 2-4 servings

Whole grains consumed 5 servings recommended intake is 6-11 servings

On the general assessment, the only things I noted were some areas of gum recession and some nail ridges. Client is very healthy and active. My client is older, so my instinct is to go with the deficient dairy intake because she could be at risk for osteoporosis. I know I can't use that because it is a medical diagnosis, so any advice on how to form my nursing diagnosis? I assume it will be risk for ..... r/t deficient calcium intake???

you can use:

imbalanced nutrition: less than body requirements r/t failure to ingest adequate dietary calcium aeb ridging of nails

the receding gums is most likely due to poor oral hygiene over time. otherwise, you could use impaired oral mucous membrance if you could find a link to the kinds of foods she eats causing irritation of the gum tissue.

if you want to link the patient to osteoporosis, you can use:

risk for imbalanced nutrition: less than body requirements r/t inadequate ingestion of calcium

and include patient teaching as part of your nursing interventions.
