Job application- past agency work

I've only done agency work the past year. It started off pretty consistent where I could get full-time hours every week for several months, but lately it's tough to get part-time hours because one hospital has hired a lot of core staff nurses and some of the hospitals have had a very low census. Now I'm looking for a job that's more consistent. The problem I have isn't necessarily the resume, but the online applications for jobs. They ask for a supervisor, supervisor's phone number, and company phone number.

I don't really know who is in charge at the agency- there is one person for payroll, someone else for scheduling, etc., it doesn't really seem like there is one person who is more in charge than the others. I've worked at four hospitals/locations this past year and at each place there were a couple of charge nurses.

What should I list under the supervisor's name? "Xyz City Office", the name of a staff person from the local office I talk with, or the name of one of the several charge nurses I've worked with at a facility?
