Nursing plan of care for hypotension/anemia

Trying to find the right nursing diangosis for patient who is anemic (H&H 9.5, 28.6 and RBC 3.31), blood transfusion x2 yesterday with hypotension and colon cancer, colostomy. What would be the main nursing dx for this patient?

Risk for Injury and Activity Intolerance come to mind...

Always consider the pathophysiology of the problem at hand, lots of causes of anemia. What's hers? Take it as a chance to figure it all out. What was/is she doing recently/currently?

Chemo/rad?

surgery?

In school I approached this as a medical student might, more than I did a nursing student. Nursing dx is the quickie little final touch. You really want to know the above so you can paint a picture of your patient. Then you can easily toss a nursing diagnosis on top of the cake.

Patient is on chemo.

Well, the plot thickens... what kind of chemo? What are the effects of that kind of chemo? Where is she on her planned course of tx? Look at labs. Your assessment - what did you find?

Then you need to look into the surgery, and at the colostomy too. Anything there?

That's all I'm gonna say, 'cause you see how there are many things you can look into?

Hmmm....Risk for Infection? Don't forget psycho-social aspects too.
