Mosby's Nursing care plans 5th edition
I used this book last year and I am gpoing to use it this year too.

whe i was in a nursing school this book was great for nursing care plans:
nursing care plans: guidelines for individualizing patient care (book with cd-rom)
by marilynn e. doenges, mary frances moorhouse, alice c. geissler-murr

if you have a little time, there are good deals to be had in the yahoo nursing student groups (my best experience was with nontradnurses) and on ebay or half.com.
