Hi ,

Am a pharmacist working in in-patient pharmacy , we are trying to create a list of continuous infusion medication that should be infused through central or peripheral line but i didn't find a reference for all medications . My question is what factors affects the decision to infuse through central or peripheral line and is there a helpful reference to check this information?

Continuous fluids that should go through a central line are TPN, I can think of others that are intermittent only but need to go in centrally such as +20meq of potassium and certain chemos are best through centrals

TPN and chemo, definitely.

Calcium chloride as well

Specializes in PICU.

We run all our pressors except dopa and doubt centrally in my picu. We can start peripherally in an emergency until we gain central access. We try to do the same for bicarb and anything w >d10 related to some unfortunate events.

Of chemos, vesicants MUST go through a central line.

Then again, there are insurance companies that seem to disagree with that line of thinking.

Concentrated K+ goes through a central. Remember that D5 1/2NS with 20K (which is very diluted in 1L fluid) goes peripherally all the time.
