Nursing home nurse practitioner

I am due to graduate as a FNP in December. I currently work as ADON at a long term care facility. They do not want me to leave and are willing to develop a position for me at the facility as a FNP to work full time 40 hrs a week.

This is a new concept to both myself and the facility. I'm trying to put something together as to exactly what my job description and duties would be so I can present it to them but I am kind of at a loss.

Our medical director is in charge of probably 60% of our residents and has no problem giving me full reign of their care but I would like to kknow exactly what I could do that would constitute 40 hours of work.

I offered to continue some of my ADON duties also so they would kind of be getting a two for one deal. I enjoy the administrative end of my job and would not mind doing this.

Any suggestions?

Personally, I found it hard to move to a different role in the same place where I worked as an RN. They still considered me an RN even though I had a full time job - APN! (just my $0.02).

Now - when I was doing clinicals, I did a couple of weeks with an FNP in a NH. It is a large NH (300 beds) and county run. She covered the entire facility and checked on residents who were sick/falls, etc.. She also did a little infection control research to ensure no trends developed such as multiple infections in the same hall, that kind of stuff. She also assessed wounds but referred the actual care to the wound care nurse. She had no administrative duties. She also went to careplan meetings and trouble-shot medical care issues with families.

I know the company that runs all the Manor Care facilities in the midwest has APNs in their NH. They don't always have just one FNP for one facility - she might cover 2 or 3 small NH in an area.
