Nursing Diagnosis - Risk for Fetal Injury

Published

My instructor wants me to use Risk for Injury R/T dependence on sole caregiver. Only mom was present postpartum. She had a c-section so she was in pain and slower. Baby had no problems (labs and assessment all normal). My question is what would be a SMART goal that I could use? Thank you!

A SMART goals:

S - Specific - So simply and clearly define what the pt is going to do

M - Measurable - It needs to be something that is measurable. That you can have tangible evidence that the goal was accomplished.

A - Attainable - Something that the pt can achieve easily without over-stressing her

R - Realistic - Something that the pt can achieve with her current situation and circumstances

T - Time - Set a time-frame for the goal to be completed.

So, something like "the neonate will remain free of injury until the day of discharge." So, it is specific...it is stating what exactly the pt is going to do (remain free of injury). It is attainable. It is measurable - has the pt sustained an injury or not? It is realistic - you don't want the pt getting injured and with the proper interventions it can be done. It has a time-frame - by day of discharge.

You can use something about the mother being able to state what to assess baby for by time of discharge (so when mother takes baby home she will know what things to watch out for and actions to take)

Mother will be able to demonstrate how to correctly place car seat in car and place baby in car seat by time of discharge.

Mother will be able to state ways to keep baby from being injured (supporting head, not placing on high surfaces unsupervised, monitoring any interactions with other children or pets, keeping hazards out of the environment, etc.)

What are your ideas?
