Nursing diagnosis help

Hi everyone

I have a case study to complete on a pt with Congestive heart failure/COPD/osteoarthritis/osteoporosis. I'm stuck on the expected nursing diagnosis' for them.

This is what I have so far:

Impaired gas exchange,

Ineffective airway clearance,

Ineffective breathing pattern,

Anxiety,

Chronic Fatigue,

Chronic Pain,

impaired physical mobility,

risk of falls,

risk of impaired skin integrity,

decreased cardiac output,

Activity intolerance

Episodes of dyspnea,

Potential for fluid volume overload,

Altered cardiopulmonary tissue perfusion ,

Risk of constipation

Self care deficit -grooming/bathing

Imbalanced nutrition, less than body requirements

Can anyone think of anything that I have missed?

chris, i don't know why you say you are stuck. that's an impressive list. unfortunately, a nursing diagnosis is merely a name for a nursing problem which has a much broader definition. a nursing problem is based on a patient's response to what is happening to them. all you've told us is that the patient has congestive heart failure, copd, osteoarthritis, and osteoporosis. these are medical diagnoses. your question is no different than if a medical student said, "i have a case study to complete on a pt with nursing diagnoses of impaired gas exchange, ineffective airway clearance, ineffective breathing pattern, anxiety, and chronic fatigue but i'm stuck on the expected medical diagnosis' for them." you'd laugh behind his back and wonder what medical college he went to. there is a process we use for diagnosing. it is called the nursing process. it involves first thoroughly assessing the patient and then going through all the signs and symptoms of their diseases and conditions because they form the basis for the nursing problems that that they have. every nursing problem has a group of signs and symptoms. since you have provided none of that information there is no way i can know if the list you provided is correct or not for this patient.

COPD:

failure to thrive r/t disability

hopelessness r/t chronic illness

Heart Failure:

death anxiety r/t terminal illness

ineffective tissue perfusion r/t decreased cellular exchange

osteoporosis:

disturbed body image r/t negative self image

social isolation r/t altered state of wellness

Sorry, I didn't give enough information.

Here's what I should have said about the case study:

A 78yr old pt diagnosed with CHF, COPD, Osteoarthritis, Osteoporosis

He is a thin man who appears malnourished. Has smoked for many years. Can only walk short distances with a frame. Is stooped and breathless. Fingers are clubbed and lips/fingertips have a bluish tinge. Lives alone but is visited by daughter once a week. Has been admitted to hospital with acute confusion and a productive cough that is producing yellow/brown sputum with an offensive smell.

Identify the expected nursing problems and suggest nursing interventions that could be inititated.

Given the above info, if anyone can think of something I may have missed in my original post I would greatly appreciate knowing!

Acute Confusion
