nursing diagnosis help GI Bleeding

Help!! My 1st care plan. I have to pick 2 problems. I dont know if the wording is correct or what the fluid volume increased relates to. I was told not to include Doctor diagnosis. Gastrointestinal Bleeding R/T Hemorrage AMB Blood in Stool and Increased Fluid Volume R/T _______ AMB JVD and Edema.

You would need to present your nursing assessment in order to formulate any nursing diagnosis. Gastrointestinal Bleeding is not a nursing diagnosis.

Yep. A GI bleed would be a medical diagnosis.
