Nursing diagnosis chart

* We have to make a nursing diagnosis chart using ADPIE, I'm not sure where to start.

An adolescent teenager comes into the emergency department with complaints of difficulty swallowing,

and large amounts of thick yellow mucous that is hard to cough up. The teenager states she was out in

the woods and was stung by a bee. When she got stung by a bee, she fell and has a large left arm

laceration.

Vital signs: HR 110 beats per minute, BP 100/70 mmHG, RR 35 breaths per minute, temperature 100.3,

and pulse oximetry of 88% on room air.

The nurse observes that the client is sweating and experiencing dyspnea and shortness of breath. The

nurse observes that the left arm laceration and a moderate amount of bleeding, redness, and swelling.
